When any other Sega Mega Drive cartidge is inserted, background characters with a blue tint aren't shown to have animation on the title screen and the code of single Blue Sphere stage is shown. The scrolling text will be replaced by "Get Blue Spheres!". When the original Sonic the Hedgehog is locked on to the Sonic & Knuckles special cartridge, a screen appears with the Sonic & Knuckles logo, models of the characters Sonic, Tails, Knuckles and Robotnik, and a line of scrolling text that says "No Way! No Way! No Way! No Way?" While this screen is visible, pressing, and at the same time changes the screen to the Blue Sphere menu.

To complete the stage, you must go around the stage and collect all the blue spheres while avoiding the red ones. It is identical to Blue Sphere, the Special Stage of Sonic 3 & Knuckles, and thus the rules are the same. This is the main type of Bonus Stage that is played in Mania Mode.
